This study explores the application of the Collaborative Chronic Care Model (CCM) to improve healthcare coordination for veterans with mental health and substance use disorders (MHDs and SUDs) as they transition from incarceration back into the community. It identifies the challenges faced by veterans and reentry support organizations, such as lapses in medications, insufficient coordination between correctional, community, and VA systems, and veterans’ anxiety about reentry. The findings emphasize the importance of redesigning work roles, providing self-management support, and enhancing organizational leadership to improve long-term health outcomes for justice-involved veterans.

Dr. Bo Kim is a researcher at the VA Center for Healthcare Organization and Implementation Research. He specializes in healthcare coordination for justice-involved veterans. This is his first work published in a peer-reviewed journal.

Allison Brown holds a BS degree in Criminal Justice with a Psychology focus and a Master’s degree in Social Work from Loyola University Chicago. She is a Licensed Clinical Social Worker who currently works at a Long-Term Insurance company bringing clinical experience to claim processes. Additionally, she developed and continues to lead the Continuing Education Program for Social Workers and Nurses and often leads complex process improvement projects Prior to this, her primary focus was in mental health and worked in both the outpatient and inpatient settings providing support to adults with mental illness.
